Background: Schizophrenia is a chronic neuropsychiatric illness with a pathological imbalance of several types of neurotransmitters such as dopamine, glutamate, and serotonin. Although drugs continue to serve as a treatment basis, optimizing patients' conditions requires switching from the standard care practice to the concept of Holistic Management. The article describes the role of the multidisciplinary team in terms of providing integrated patient management: Clinical Psychiatry ensures accurate diagnosis and a general medical treatment strategy; Clinical Pharmacy contributes to proper medication adjustment and prevention of adverse drug interactions; Nursing Support offers psychosocial support and regular physical condition evaluation; and Biochemical Laboratory conducts regular monitoring of patient biomarkers and therapeutic drug toxicity. Method: Systematic narrative review of peer-reviewed articles from PubMed, Scopus, Cochrane, and WHO databases for the period from 2020 to 2025. Result: It was proven that collaboration between psychiatrists, clinical pharmacists, psychiatric nurses, and laboratory scientists helps decrease the rate of hospitalizations and increase the adherence to medications (50–80%). Moreover, regular biochemical and metabolic monitoring leads to better quality of life and reduces cardio metabolic mortality related to second-generation antipsychotics.Downloads
